I have the same tank on my 250 bitsa, so I can’t say what model it was originally fitted to. I will try and get you locating dimensions for the rear hole tomorrow. Yes, the rear hole is offset from center. The wires exit the hole to the left side of the fender, from behind the fender loop.

I think that one of the two holes called out on nalimugmug’s photo is the forward hole for securing the fender to the frame near the battery box.

The wiring exits the fender near the taillight just behind the fender loop.
